Follow-up Coaching Session

Service Description

Follow-up Sessions will take place after the Discovery, Initial, & Goal Setting Sessions. A minimum of 3-6 months commitment to health & well-being coaching is recommended to achieve sustainable health & well-being behavior change. For clients with significant or multiple health & well-being concerns, 6 months or longer is recommended. Please complete Follow-up Coaching Session Prep Form prior to follow-up sessions. A fill & sign version of this form will be sent to you weekly after the Goal Setting Session. Session times will vary, plan on 25-35min for this session.
